 Pilot Experience Form [PDF 44KB]
This form is designed to capture a pilot’s general information, flight experience and training/certification record. The pilot experience form is to be filled out in conjunction with an aircraft insurance application for all named pilots on the policy.
Some Important Information
In the general pilot information section it is important to include, if applicable, maintaining membership in an aviation organization such as Aircraft Owners and Pilots Association (AOPA) or the Experimental Aircraft Association (EAA). Some aviation insurers will offer a discount to premium based on a pilot’s membership in either of these two organizations. Membership numbers are required to be noted on the pilot form to validate participation.
Of particular importance to include in the pilot’s flight experience section is the recent flight experience – last 90 days and last 12 months. If applicable, flight time should also be disseminated by fixed, tailwheel (conventional) or retractable gear versus float or amphibian experience. Flight time in the make and model of aircraft to be insured should be noted in the area provided by total time in the make and model and total time during the last 12 months.
The three areas which are noteworthy to assessing pilot risk in the aircraft to be insured are:

Total logged hours in the same make and model of aircraft to be insured.

Total logged hours during the last 12 months in the make and model to be insured.

Is recurrent training received in the make and model aircraft to be insured?
Please indicate any recurrent training that is received to include the make and model aircraft in which training is received, date of training and appropriate training school.
Lastly it is imperative to disclose and provide brief details of any Accidents/Incidents, Waivers or past Violations which may be on a pilot’s record. This information should be noted on a separate piece of paper if the form is to be faxed to Wings. If the form is to be mailed it is ok to include details on the back of the form.
